Progress Notes

Progress notes are the written record of what happened on a shift: goals worked on, outcomes, mood, and any follow-up. They are the main piece of evidence the NDIS Commission and plan managers rely on during audits.

How it works
  • Use "New progress note" to log a note against a participant and (optionally) a specific shift.
  • Capture a summary, goals addressed, and any risks observed. Keep it factual, objective, and dated.
  • Notes are timestamped and author-stamped automatically and cannot be deleted after 24 hours — edits create an audit trail instead.
Tips
  • Write notes the same day the shift occurs; late notes reduce evidentiary weight.
  • If an incident occurred, log an Incident Report as well — the two records cross-reference each other.
